CASE MANAGEMENT - OLDER ADULTS
Provides assessments and develops care plans for older adults who are in need of assistance with coordination of services. Provides a complete assessment of needs by a social worker, nurse, and/or mental health counselor. Case manager coordinates the delivery of services, ensures that the services are obtained in accordance with the case plan, and follows up and monitors progress.
